The Registration Process We have designed this page to help you with the Registration Process and to explain in further detail the information we are requesting from you. You will need Adobe Acrobat Reader to view & download both documents. Please download & print both documents before reading this page.   The Student Registration Contract Download, click The left side of the document relates to the payment process, and what forms of payment we accept. Starting at the top of the page, please fill in the TOTAL PROGRAM FEE amount, along with the DEPOSIT AMOUNT for the specific tour you are registering for. Below this section is the PAYMENT TYPE section. Please choose the method of payment you will be using to pay for the tour. We accept personal checks (made out to NIEP College Preview), VISA & Mastercard credit cards. Unfortunately, at this time, we do not accept American Express or Discovery. You may pay a deposit with a personal check and the remaining balance by credit card or vice versa. The right side of the document relates with information regarding the student participant attending the tour. Starting at the top of the page, please fill in the name of your High School, and the name & dates of the Tour you would like to attend. Below this section, please fill in the name & address where you normally receive mail. It is very important that you list the correct address in this section, since you will be receiving information about the tour (meeting places, final schedules, contact information, etc.) and billing information. After this section is the PARTICIPANT HEALTH RECORD section. Please note your health status, and if you are currently taking any “prescription” medication, you do not have to list any over-the-counter medication you are taking. If you are taking prescription medication, it will need to be given to the NIEP Courier before the tour starts. Finally, please list your Health Insurance Company & Policy number. No student will be allowed on the tour without this information. The next section is the ROOMING REQUESTS section. Normally rooming assignments consist of (4) students per room, (2) students per bed, same sex rooms. You may request your own bed (an additional $50 charge per night) or your own room (an additional $100 charge per night). If you have a roommate preference, please list the name(s) in this section. Finally, at the bottom of the page is the AUTHORIZATION & SIGNATURES section. We require the signatures of both the student participant and the Parent/ Guardian. We will not allow a student to attend the tour without a signed document   The Parent Permission Form Download, click Starting from the top, please re-enter the Medical Insurance information for the student participating in the tour. This information should match what is listed on the STUDENT REGISTRATION CONTRACT. Again, it is very important that this section is completed correctly, failing to do so could lead to the student participant not receiving immediate medical attention if injured. Below this section are listed the (3) types of adult supervision NIEP will provide on the tour. Selection (A) is the most standard level of supervision provided, and the most common choice for most student participants on the tour. You are allowing your child to leave the main group with other group participants, but he/she must obey the rules and instructions established by the NIEP Courier, failing to do so can lead to expulsion from the tour. Selection (B) allows the student participant to leave the main group to meet with family friends or relatives during the tour. Contact name(s) and phone numbers must be provided, and the student participant will not be allowed to leave the group without the NIEP Courier meeting the responsible party in person. Selection (C) is the most restrictive and inflexible level of supervision provided. The student participant will not be allowed to leave the main group without the NIEP Courier being by his/her side at all times. Please note that NIEP College Preview STRONGLY discourages parents/guardians from selecting this option. Finally, at the bottom of the page, is a section for student participants that have alternate travel plans & will meet up with the main group on their own schedule. For example, if you are a student traveling from New York to attend our Southern California Colleges Tour, please list your airline & hotel stay information, along with how & where you will meet up with the main group at the start of the tour. Also, if you require travel assistance at the end of the tour, please note this as well. Lastly, at the bottom of the page, the Parent/ Guardian must sign & date this form.
